Adult Day Center

Job description:
Responsibilities:

  • Assessing the client’s nursing and medical needs;
  • Developing a client’s individual plan of care;
  • Assessing, observing, evaluating, and documenting each individual’s health condition, and instituting appropriate nursing intervention to stabilize or improve an individual’s condition or prevent complications;
  • Ensuring that the individual plan of care appropriately reflects the physician’s orders; counseling the individual on their health needs or illness and involving significant others in the discussion of their immediate and long-term goals; and
  • Providing or supervising personal care services to enable the individual to restore, maintain, or improve their ability to perform personal care tasks  (i.e. dressing, feed, grooming, bathing, toileting, transferring/ambulation, or assisting with self- administering medication)
  • Obtaining physician’s orders for medication and treatments to be administered;
  • Entering, dating, and signing monthly progress notes on medical care provided;
  • Administering medications and treatments;
  • Providing health education; and
  • Maintaining medical records
  • Conducting a health assessment/plan of care to determine whether the applicant appears to have a medical need for the services; by completing DADS’ Clients Health Assessment/Plan of Care form; and
  • Notifying and maintaining communication with DADS and insurance companies.
  • Supervising CNA direct care staff scheduling and time off requests.
  • Assigning clients to appropriate units.
  • Working with Activity Director to assign CNA staff to appropriate units.
  • Working with Director and Assistant Director in hiring, employee counseling, unit assignments, and disciplinary actions.

Minimum Experience & Other Requirements:

  • Must be willing to participate in on-going training
  • One year of experience in a human service or medically- related program
  • Preferred experience with individuals with disabilities and/or the elderly

Licensure Required:

  • Must pass all required background checks
  • Must be a licensed vocational nurse (LVN)
  • Must have a current license from the Board of Vocational Nurse Examiners for the State of Texas
  • Must practice in compliance with the Vocational Nurse Act and rules/ regulations of the Board of Vocational Nurse Examiners
  • Must maintain CPR certification

  • Adult Day Center Director
  • Adult Day Center Assistant Director

Pay: $12.00 – $12.50 per hour
Job description:
Responsibilities: Provide personal care services, including:

  • dressing
  • feeding
  • toileting
  • vital signs
  • ambulation
  • exercises
  • Assist with recreational activities,
  • Encouraging clients to join activities,
  • Provide protective supervision,
  • Document care provided to each client and report all changes, both physical and mental, to the nurse, and
  • Embody the values of integrity, professional teamwork, and compassionate service every day.
  • Anything else that needs to be done
  • Work front desk, signing clients in and out
  • Help with cleaning

Minimum Education and Experience Required:

  • CNA license (Preferred)
  • CPR certification (training provided)
  • Clear TB screening
  • Driver’s license
  • Social Security card

Orientation and ongoing Training:

  • Initial fire, disaster and evacuation procedure within the first 3 days of employment
  • 18 hours of training during the first 3 months of employment
  • 12 hours of training each year

Dress: Scrubs, Name tag, & Closed-toe shoes

Reports to: Adult Day Care Director, Facility LVNs, & ADC Activity Director

Guardianship

Pay: $32,000.00 – $36,000.00 per year
Job description:
Friends for Life is seeking a service-minded, committed, and professional team player to provide compassionate service to support the highest level, quality of life for elderly and persons with disabilities.
Requirements:
To be eligible for consideration the applicant must:

  • Be at least 21 years of age;
  • Be a high school graduate or possess the GED equivalent
  • Meet ONE of these additional qualifications: Have two years of relevant work experience related to guardianship.
  • Have a minimum of a bachelor’s degree conferred by a college or university accredited by an organization recognized by the Texas Higher Education Coordinating Board in a field related to guardianship.
  • Have completed a course curriculum or training specifically related to guardianship approved by the Texas Judicial Branch Certification Commission.

Core Responsibilities of the Position:

  • Arrange medical and mental health care, educational training, rehabilitative, and social services
  • Maintain regular monthly face-to-face contact with clients on assigned caseload
  • Document client-related activity in the caseload in the guardianship database system
  • Assist in the management of the financial affairs of clients as assigned

Additional Eligibility Criteria for Initial and Ongoing Employment:

  • Clear state and federal criminal background checks.
  • Pass a certification exam and obtain Texas Guardian Certification within two years.
  • Possess and maintain a valid driver’s license and current automobile insurance.
  • Have a reliable vehicle available to you at all times.
  • Be willing to travel for work within the service area at least 50% of work time.

Life Skills

Pay: $12.00 per hour

Job Description:
Life Skills Trainers provide one on one skills training for persons with Intellectual or Developmental Disabilities living in nursing facility settings with the goal of increasing independence. Typical skills training includes activities that enhance socialization/ communication skills and cognitive skills; it is individually designed for each client served. No direct care responsibilities.

On the job training for the position will be provided. No certification required. No nights or weekends. Paid holidays and earned leave, even your birthday is a paid holiday. Monday through Friday but work schedule is flexible.

Successful Life Skills Trainers are creative and can work independently without direct supervision. You can really see the difference you make in the lives of the clients you assist.
